Primary Care Connect is delighted to be a partner in delivering this innovative Community Health First campaign, involving all 24 independently managed community health organisations across the state.

We believe in, and advocate for, a strong and appropriately funded Community Health sector, which is responsive and flexible enough to meet the needs of community. We are passionate about collaboration and developing our shared expertise, focused on successful and impactful outcomes for individuals and our communities, dedicated to prioritising access for those who are most vulnerable or experiencing hardship and disadvantage.

A robust Community Health sector can help solve the critical challenges faced by the Primary Health system, and provide for a collaborative and holistic approach to a range of social and health issues.
